What Information Is Required

A service request email or memo addressed to the Lab Director must be sent to servicerequest@forensics.lc or slfslrequest@govt.lc requesting toxicology services offered through the laboratory with the following:

1. The subject line of the email should read 'Service request for (type of analysis) for (CMS/Court case number/PM case number/Traffic case number). If the case is a priority, please indicate this in the email subject heading.
2. The requestor’s Name, Rank, Force number, Department/Station name and address, where applicable.
3. A brief outline of case circumstances.
4. Victim’s name (Correct spelling using national ID)
5. Victim’s Date of Birth and gender
6. A complete list of exhibits to be processed.
7. Date and time of collection
8. Sample source and specimen type
9. Type of test and test number (Consult with pathologist)
10. Specify the responsible party for settling the shipping and testing fees
